A good review isn't a formal grilling. It's a straight conversation about how someone's going, what's working, and what they need to do their job well. Done regularly, it catches the small stuff before it turns into the big stuff, and it means no one's blindsided if a harder conversation ever comes. Fill this in together, keep a copy on file, and set the date for the next one.
Start with the good. What's this person doing well, and where have they stepped up since last time?
Be straight and be specific. Talk about the work, not the person. What needs to change, and what does good look like?

Training, gear, a hand from someone, clearer instructions. What does this person need from you to do the job well?

Signing just means you both had the conversation and agree on what was said. It's the record that the check-in happened.
